Toxic bacillary dysentery ferocious, fast-changing, if not timely rescue, and soon died. How can early detection? Firstly, we need to know the early signs of toxic dysentery, while careful observation, attention to early changes in condition. Early toxic bacillary dysentery mainly as follows.
(1) sudden onset, fever above 39 ℃, irritability, delirium, use of sedatives is invalid, or lethargy, malaise.
(2) Early respiratory failure. Respiratory rhythm is not sharp, suffocation, suspension. Repeated Jing Ji, convulsions, shock just after the general situation worse. Limbs sustained muscle tension, lower extremity adduction, upper limb internal rotation, high blood pressure. Pediatric normal blood pressure for age (years) × 2 10 80 mm Hg (10.7 kPa), more than the normal 20 mm Hg (2.67 Pa dry) shall be high.
(3) Early circulatory failure. Acral cyanosis, skin Flooding; pulse pressure is small (less than 2.67 Pa dry, which is 20 mm Hg); heart rate fast, for more than 90 times per minute for adults, newborn 150 times more than 120 times over two years of age, 3 — 7-year-old more than 110 ,7-14-year-old 100 times above; urine less urine volume 25m1 per hour under adult, infant of less than 10ml, children less than 20m1.
(4) The early signs of respiratory distress syndrome. A small number of toxic dysentery, there may be respiratory distress syndrome, mortality is highest, rescue very difficult, but if early detection, mortality can be reduced to below 20%. Mainly manifested as acute, progressive, inspiratory dyspnea, respiratory effort without airway obstruction and difficult to alleviate the hypoxemia, bilateral pulmonary auscultation, no obvious abnormalities, symptoms and signs clearly disproportionate, x ray shows the transparency of the lung decreased mesh can be seen followed by a shadow, last seen “white lung.” Blood gas analysis is characterized by partial pressure of oxygen “dry 8 kPa (60 mm Hg), carbon dioxide partial pressure” 4.67 kPa (35 mm Hg), pH value is increased (later reduced).
